Lee Health Population Health Program
Hired by Lee Memorial Health System to lead the evolution of the Physician Hospital Organization from volume to value-based care principles. The Lee Physician Hospital Organization (PHO) consisted of approximately 1,000 physicians at the time.
We guided the selected physician leaders in the development of a governance structure to support the movement to value-based care, collaborated with the informatics personnel to create performance reports for the providers, and implemented a process for equitable shared savings distribution. We designed and initiated shared risk contracts with PHO and Commercial and Medicare health plan programs.
Over the next 4 years:
The program grew from 7,500 to over 60,000 covered lives.
The provider base grew from 650 to over 1,000 participating providers.
Quality measure outcomes improved to above the NCQA median for all with some at the 90% percentile
Total shared savings of over $4 million for distributions
The value to the community was improved outcomes with a lower cost and the providers were well-positioned for Quality Program Payment (MIPS-QPP) processes when they were deployed by CMS in 2017.
